To register your child in Brookhaven SDA School the following forms are required. These forms are available in Microsoft Word format and in PDF format. The Microsoft Word forms will let you type in your student information and print it out. The first time you open the Word document it may prompt you to "enable editing". You will have to click that first before you are able to enter your information in the forms. If you don't have Microsoft Word on your computer, then you may use the PDF form. Just print it out and fill out the form in pen. After completing the forms, just bring them with you to school registration.
